Option 1
 Proxy Access

I am a parent or legal guardian requesting access for a patient under 18 years old - Parent or legal guardian completes steps.

For children 14-17 years old, the health information you'll be able to view will be limited. When your child turns 18, your access will be turned off completely. Your teen child can choose to give you full access in both of these cases by completing the appropriate workflow - please see Option 3. These age limitations do not affect any legal right you have to access your child's record by other means.
At this time we are unable to grant access to minors or adolescents in Foster Care.


I have an active MyReidHealth MyChart account

4. Complete and submit the form.

Option 2
Proxy Access

I am a family member, friend or caregiver requesting access to an adult or emancipated minor's health information in MyChart.


The patient must complete Option 3 to grant you access to their account.

Note: If a physician determines a patient does not have medical decision-making capacity, an appropriate surrogate (i.e., Power of Attorney for Health Care, Health Care Representative, Court appointed legal guardian, or other surrogate under Indiana State law) may request and be granted full access to a patient’s MyChart account; provided, the individual requesting access must provide copies of any legal documents verifying the surrogate’s authority to have access to the patient's medical information.

Note: You must have power of attorney or legal guardianship.

Option 3
Proxy Access

I am a patient 14 or older and I would like to give someone access to my account - Patient completes.


1. Click here to sign in to your account.

2. Once you are signed in select the 'Health' tab and click 'Share my Record'.

3. On the Share my Record page in the 'Share with People' section, select 'Friends and family access'.

4. Complete and submit the form.

5. You'll receive a message when your invitation has been accepted. Invitations automatically expire after 30 days.

2. Once your account has been created and you are signed in select the 'Health' tab and click 'Share my Record'.

3. On the Share my Record page in the 'Share with People' section, select 'Friends and family access'.

4. Complete and submit the form.

5. You'll receive a message when your invitation has been accepted. Invitations automatically expire after 30 days.
